All students must have an Emergency Medical Form on file in the health office.  If your child is ill or injured at school, it is important that you be contacted as soon as possible.  Medical emergency forms are distributed annually in the summer or at registration.

 

If your child must take medication at school, parent and physician written permission is required.  Please complete one of the following:
A)  Prescription Medication Authorization Form -  Students with medication orders for asthma and/or food allergies must additionally supply an action plan with their medication orders. New: Medication Authorization Form for Life Threatening Allergies
 
 
 
B) Non-Prescription Medication Authorization Form- This form is for Over-the-Counter (OTC) Medications which your physician must complete.

 

If your child is in grades Kindergarten through fifth grade and has a physical examination by their family health care provider, a report form is requested  for updating school records. Please use the Physical Exam Form Grades Pre-K to 5 form.

 

The Physical Form Grade 6-8 form is a state form that is required before participation in any inter-scholastic sport.  A student’s physical examination report is valid for 365 days from the date of the physical examination. Athletes should submit this form annually.  It is strongly recommended that all sixth through eighth grade students submit this form in September.  Many students are surprised at delays in obtaining appointments with their healthcare providers for last minute physical examinations.   It may take one week to have their forms reviewed by the school physician.  The school physician signature is required before sports participation.

 

The Health History Update must be completed by a parent whenever a physical examination has been completed more than 60 days prior to the first sports practice.  Parents may expect to complete this form several times during the school year if their child is participating in several sports.
